The Company
Our client supplies private nurse led home care and due to a recent company restructure, is looking to recruit a Care Coordinator. This is an exciting opportunity to demonstrate your capability, adding value and leveraging the company with your excellent coordination skills and professional personality.

Summary
The Care Coordinator is responsible for the allocation of suitable staff into a shift rota within the local area.

Responsibilities/duties:
•Supporting the Nurse Manager to monitor each individual Service User's rota requirements and ensure that suitable carers are allocated to vacant shifts.
•Coordinate shifts via email, phone, letter and/or fax.
•Upload relevant information to the CRM/database system (knowledge of Caras would be advantageous but is not essential).
•Cross check notes written on shifts to see if there is anything to action.
•Set up default rotas for the Service Users that have regular shifts.
•Check calls are made to Service Users and Staff to check compatibility.
•Update Carers compatibility list.
•Distribute rotas fortnightly.
•Check 'Easy Tracker' is set up for all Service Users that require it and that staff are set up with a pin number.
•Ensure all carers being introduced to a Service User for the first time, are given a thorough briefing using the Service User's care plan.
•Deal with queries and enquiries.
•Record important notes/information on CRM/database system.
•Action any rota changes requested immediately
•Sensitive information is accurate and up to date on the CRM/database and filed away.

You:
•Nursing, caring or healthcare experience is desirable but not essential
•Rota and shift organisation is desirable
•Exposed to coordinating people to jobs/activities/services/maintenance
•Attention to detail
•Happy to work a call rota system
•Effective professional people skills
•Maths and English GCSE or equivalent
•Confidence sharing important information with clients, customers and the general public
•Computer literacy
