We're looking for an enthusiastic, approachable and solution focussed individual to work within a multi-disciplinary team, delivering ongoing assessment and rehabilitation and training services to our beneficiaries in their homes and communities.

You'll assess our beneficiaries' need for advice and information, rehabilitation and training, care support and financial assistance. Our aim is to work in conjunction with statutory and voluntary agencies to help our beneficiaries maximise their independence.

The ideal candidate will have a positive, collaborative approach and will be experienced in following a rigorous case management process, while always considering safeguarding, risk management, confidentiality and professional boundaries. You'll be aware of resources and benefits available to older people and people with disabilities and will have (NVQ3) in Health & Social Care or equivalent relevant social care experience.

A working knowledge of Microsoft Office, excellent organisational and communication skills and the ability to travel to any part of the region, some of which may be inaccessible by public transport, are also requirements.
